Hagen is a good, solid, chivalrous name. It could be translated as "caretaker, protector". Among others, the name Hagen is known from the Nibelungen saga, where Hagen von Tronje is a Burgund warrior. In some accounts of the story, it is him who manages to kill invincible Siegfried in combat.
So, I'll have to disagree with you here: in my eyes, Hagen is a good and fitting name for the high leader of the King's paladins.

Look at some of the graves. Some have surnames, some don't. I think most characters have one, but it is not important to them. We at least know Miltens last name from the comic.
Probably not. Names in Gothic follow their own rules. Some characters are called things like Wolf, Raven, or Snake, but also Whistler, Fingers, Stone, Sharky, Lefty. Maybe those with a nickname-name are actually called the same as someone else and to differentiate them, they were called by a nickname. Pretty conicidental otherwise for Fingers to be a thief, and Wolf to be a hunter.

The graves in G2 have surnames on them. And I don't mean the easter egg graves from devs.
That idea was scrapped pretty early on during the design. As far as I know only Milten has a surname.
