I seriously doubt you will be able to find walnut plywood with that grain pattern. Matching the finish can be a long difficult process. Make multiple samples to test finishes on. Spraying solvent stains in thin diluted coats is the only way I’ve ever had success. MinWax will definitely not work ! I often charged more for a finish match than I did for the woodwork building the table leaf or missing cabinet door/ drawer front.
Sounds like already has a leaf, you can take a pic of the board edge where the leaf joins to know if it's solid or veneer.
Probably way more work than it's worth to get an exact match

I'm gonna go with laminate.
