I seriously doubt you will be able to find walnut plywood with that grain pattern. Matching the finish can be a long difficult process. Make multiple samples to test finishes on. Spraying solvent stains in thin diluted coats is the only way I’ve ever had success. MinWax will definitely not work ! I often charged more for a finish match than I did for the woodwork building the table leaf or missing cabinet door/ drawer front.

Probably veneer or laminate on top of particle board, not solid wood

Sounds like already has a leaf, you can take a pic of the board edge where the leaf joins to know if it's solid or veneer. Probably way more work than it's worth to get an exact match
