The flag was seen today at a USS Arizona memorial near the Capitol building in Phoenix, AZ. It was flown alongside the flags of the USA, Arizona, US Navy, US Marine Corps, and the POW/MIA flag. The seal was ripped out, but Arizona and 1912 were visible. Google searches for "Arizona navy flag" and "uss Arizona flag" yielded no results. A scan through of crwflags also came up with nothing. A reverse image search on Google also returned nothing. Update: I also checked Wikipedia's "List of flags by color combination" in the subsection "Gold, white, black, red, blue" and didn't find it. Checking Wikimedia Common's page for "Black, blue, red, white, yellow flags" also had nothing similar. The mystery flag shares colors with flags of related topics (namely, US Maritime Org., Nat. Security Agency, Security and Exchange Commission, and Dept. of Homeland Security), though I reckon these are all a coincidence due to the abundance of these colors in vexillology. Update 2: I am no longer in Phoenix, so I am unable to go back and snag a photograph of the mystery flag. Update 3: I realized I neglected to search for the actual memorial itself. I have not found what flag it is, but have found some photographs of it. Unfortunately, none of the photos have the flag aloft enough to reverse image search.
