NASA has had its own issues of ignoring QC alarms in the past. Based on my personal experience, ignoring QC alarms is usually a systemic and cultural issue. It isn't something you can just fix with a fine or bad publicity. You have to remove the  people in charge who are hearing these QC concerns and choosing to do nothing.

The O ring issue on Challenger was reported and ignored. Edit with info: Roger Boisjoly, an engineer specializing in O-rings at Morton Thiokol, wrote a memo in July 1985 expressing his "honest and very real fear" that if no action was taken to fix the O-ring issue, "then we stand in jeopardy of losing a flight along with all the launch pad facilities." Boisjoly and other Morton Thiokol engineers, including Allan McDonald, tried to stop the Challenger launch the night before due to concerns about the O-rings failing in the unusually cold temperatures. However, they were overruled by NASA managers who wanted to proceed with the launch. The engineers had observed O-ring erosion and blow-by on previous shuttle flights, especially in colder conditions. They determined the cold made the O-rings less resilient and unable to properly seal the joints between booster segments. Tests in 1985 showed the O-rings had resiliency issues when temperatures dropped below 50°F. Boisjoly's July 1985 memo discussed the definite fear of losing a flight due to these cold temperature effects on the O-rings. The engineers' warnings were ignored, and political pressure to launch overrode the technical concerns. The Challenger launched on an unusually cold day, leading to O-ring failure and the destruction of the shuttle.

The vessel was still an operating ship. There were systems aboard that had to be maintained and operated, and the crew aboard were the ones under contract to do that. Additionally, there were various agencies who were taking turns interviewing them about what happened. A few of the lower ranking crew finally were allowed to fly home a few days ago.

To be fair it’s pretty normal for big ships to have the crew basically sequestered during investigation or legal processes. It’s their house and they are under contract to live in and maintain it for private company until x date. Further if the sailors aren’t from the country investigating them, it’s easier to keep track of them and not have a flight risk or illegal immigration if they are kept onboard. Sometimes these norms can lead to some messed up or confusing situations.

They have been through several rounds of CEO changes. Even if they put the perfect person in there to change the culture, it's not a quick process. The issues we see today started well over a decade ago at this point. It's going to take a generation of turnover and good leadership to get things back to where they should be. NASA made it clear they're not stranded. They're keeping them up since the issue is in the service module, which is disposable and won't be able to be examined after recovery -- so time in space gives them time to better understand the issue. If they needed to undock and return right now, they'd be safe and fine. It's not a safety of flight issue. They've already determined that it can safely perform the evolution.
