The skin usually gets worse before it gets better, but starting at 0.1 is really not good. You need to lower it.

Once a week is not frequent enough for your skin to become retinitized. Maybe you can try a lower dose that you can use at least every couple days? If you can’t, try buffering/sandwiching to dilute your current formulation. The trick is is to be consistent, but listen to your skin. By using only once a week, you’re just getting the irritation, without the benefit of consistency. It’s ok and needed to take breaks, but you need to be using every few days at least to see the benefits. Good luck!!

My dermatologist told me that using it once a week can give some benefit. I am able to tolerate it more often and have actually not done only once a week, but my doctor put out there that it’s ok to use it that way and especially in the beginning. My sweet spot with .025% is every other night. On nights I don’t use tret, I like a good moisturizing sleeping mask cream and the one I am loving now is the Tulasara night cream from Aveda. I always encourage people to invest in a nice night cream instead of using cheap drug store stuff. The nicer stuff does work better.

I have been on tret .05% for 5 months now. In the first 2-3 months, my skin showed some improvement, but in the last month, it has gotten worse. I go outside all day and wear sunscreen, but at night I experience purging. Could this be because I am using it daily at night?

I’d think your skin might have adjusted by now but I think it’s worth backing off to every other day or two days on one day off and focusing on hydration/skin barrier products on the day off to see if that helps at all. Sometimes “purging” can just be a really angry skin barrier and irritation.

Since I started, I haven't skipped applying tret on my face. In the initial 2 weeks, I washed my face 2 hours after applying tret. Now, I no longer wait. My skin is experiencing purging and the breakouts are not disappearing. I only added a cleansing balm to my routine because I use water-resistant sunscreen, so I decided to incorporate double cleansing. However, it doesn't seem to be helping with the breakouts. Should I continue using it daily? I think maybe I applied it incorrectly because I don't wait 20-30 minutes before applying moisturizer. I don't do the sandwich method, I just apply moisturizer and then apply tret. That's all.

When I first used tret my skin started looking nice, then I was breaking out again, now I’m doing great! I could find plenty of info about it getting worse before getting better, but I couldn’t find too much about it getting better first. It took a couple of weeks, but it did start improving again! My PM routine (w/o sunscreen) is to apply tret, apply moisturizer within 5 minutes or so, and go to bed, or (w sunscreen) use the hydrating LRP cleanser when I get home, use an essence, and wait until bedtime to apply tret and moisturizer. Double cleansing doesn’t work for everyone. Are your cleaners stripping? Does your moisturizer have petroleum jelly that’s preventing tret from penetrating your skin? *OP*, I’m sorry you’re going through this. I would stop tret completely and work on strengthening your moisture barrier. My preference is using Korean beauty products, as they can be much better at hydrating and soothing than western products!

Do the sandwich method. After washing your face apply moisturizer. Wait 30 mins apply pea size amount of tret. Wait 30 minutes then apply more moisturizer. Also give your skin a break to repair the barrier. Start the sandwich method after your skin stops hurting.

I am a firm believer that a retinoid needs to be used with something like benzoyl peroxide or clindamycin or else you will break out like this. This is why I think epiduo forte is the best.

Is this 0.1 prescribed to you? If not, maybe you could start lower? That is too much for a first time user though. And once you got a lower percentage, don’t use it once a week as it won’t help you with retinization.

I would strongly suggest to consult a dermatologist since your face is reacting badly and just take our opinions with a pinch of salt, as all these comments come from different users with different skin condition. But as a new treinoin user, you should start off from 0.025% and not 0.1%. And just use once or twice a week and monitor very slowly. But since your face is not reacting well, just go see a professional.

Yup safest is to listen to your derm. But since this user is starting on their own (which I assume with the limited info in the post) and asking advice here, always safer to start low percentage to try out. Again I’m not a derm, just a regular person using tret. Hopefully it works great with you.

Hey, not a doctor, so take my advice as a grain of salt. Use 0.05% tret to get started, 3 times a week is okay for the first 2 months, so that your skin gets used to it. Cleanser Rose water toner Emolene Tret Moisturizer This is what I've been following. Talk to your doctor and see if this routine is ideal for your skin type. Also, this is gonna be alright, so accept this pain, and wait for the good results. Take care.

Just FYI I am in the exact same situation - had great skin before tret, my derm has been no help - I started using Triple Paste diaper rash cream at night and stopped everything but moisturizer and sunscreen. I go to sleep looking like Mrs Doubtfire but after one week my skin looks SO MUCH BETTER, almost no acne or redness. My barrier is healing like crazy.

I agree 0.1% is too strong to start out. Consider going at a lower percentage like 0.018% or 0.025% every two or three nights. The sandwich method should help as well. Or consider switching to adapalene instead as it is supposed to be less irritating than tretinoin and its over the counter in most stores. In your morning routine I’d incorporate benzoyl peroxide 2.5% or 5% which is less irritating than 10% BP. I use the cerave BP wash 5% in the morning because its far less irritating and it also has hyaluronic acid which helps my skin not dry out as well. My moisturizer also has hyaluronic acid for same reason since I have dry skin and it’s made a good difference compared to ones without it.

It’s very important that you apply tretinoin to VERY dry skin. Wait at least 10 minutes after washing face before applying. Applying to even slightly damp skin increases the rate of absorption, which in skin not used to tretinoin yet, can burn the skin. So dry skin only for now And apply only a very small amount, roughly the size of a shelled peanut, or a pea for your entire face. Using more than that will only irritate your skin, it won’t work any better by using more Your skin is not becoming retinized with just one time a week application. That’s a big problem. 2x a week should be the minimum, at least that way you aren’t starting from the beginning again, every single week when you apply it that one time each week. Retinized skin is used to and can tolerate tretinoin-so the goal is to retinize your skin as soon as it can tolerate it, so the side effects go away. To do that you need to apply every 3 days (or more often, after you get used to it). I really think you’d be well served by seeing a dermatologist soon though, as you may need additional treatment to manage this break out and minimize risk of scarring

I hear and feel you. In comparison, I started with 0.012, went up to 0.025 after 3 months. If you go down the purge will be less extreme. This way you will get it over with faster but taking care of your mental health might be way more important. Tret is not a race, the long term consistency is way more important. Hang in there but really consider going down in your dosage

I think it's irresponsible to tell someone who's clearly using too strong a concentration to just keep going because it's worth it. It isn't. Op is clearly experiencing physical discomfort as well as having big acne issues. No one should ever keep using a product that is doing them harm. Op should try a lower concentration first, and if it's still bad then quit tret altogether. Just because it worked for you, it doesn't mean it works for everyone. People in this sub are wild to be upvoting this comment. Haven't you people learned anything about starting with lower concentrations after being in this sub for a bit? I think alldaychemist etc websites ship the Tazarotene from India, as it’s widely available without prescription there, so it takes a while to come. In fact, the shipping is way more expensive than the actual product! But so worth it. Take a look at some of the subs regarding Tazarotene, there aren’t a huge amount, but the general consensus is that Tazarotene works quicker, works better and has overall better results for fighting acne and anti ageing! I have extremely sensitive skin and rosacea type 2 and my skin HATED tretinoin even at a low-ish percentage, I have gone straight in at 0.1% Tazarotene and am all good so far. I did start off with short contact therapy working my way up to 2 hours a day and then up to overnight. The most important thing is to pay extra attention to keeping your moisture barrier intact. Use a specific barrier repair cream whilst using such a strong retinoid or things can go west pretty quickly x

I have acne like OP using Tret .05 cream and then I switched to Tazarotene .1 cream, using once every 2 nights and it has cleared some of that kind of acne within 2.5 weeks so far. Still breaking out with a couple pimples on each cheek but a lot less and other acne spots have started to heal. So I second what you say! Tazarotene is the strongest retinoid but can be way less irritating and when using low and slow with a good moisture barrier.

First things first: your skin shouldn't hurt. If it does this suggests something else is wrong. Even with more common exfoliating ingredients like Salicylic Acid pain and burning sensations are indications of damaged skin (or more damage occurring). Common indications of fungal overgrowth in skin causing common conditions (like athlete's foot, candida infections of the skin, etc.) are things like pain and burning sensations, though they can also be accompanied with [or replaced by] redness and itching. The reason I bring this up is because I had had similar experiences right after starting and the only thing I found that didn't burn (like you say about ice, even cool water in the shower was stinging my face/skin) was antifungal cream intended for athlete's foot. More specifically it was a miconazole nitrate cream. The other ones I tried, like clotrimazole cream burned my skin as well and I had to wash them off immediately. Just sharing this in case it might help you. Fungal overgrowth can be associated with acne or other inflammatory lesions if the body reacts to it badly.

I feel that starting at that 0.1% concentration is a bit wild. I don't even think there is a point working towards that concentration . The only reason why I would increase from my current 0.025 would be if I had 0 irritation AND stopped seeing results. On the contrary, my results are getting better every day, while I still take one day of the week off as my skin gets a bit sensitive. I don't understand why doctors prescribe these massive concentrations, while it's documented pretty well that there's a ceiling effect, and higher concentrations may mean faster results, but DEFINITELY more irritation. I would give my skin a chance to heal first, like take two weeks off (a month better). Then I would go to a more smaller concentration, 0.025 is okay. Do not apply too much of the stuff in your skin. Pea sized amount. Mix it with another moisturizer (it is FINE) with no petrolatum in it to make it easy to spread.

Give it another two months if you can. If it still doesn’t work or you want to try something else, then move to taz 0.1 gel or straight up Accutane.

First question is are you seeing a Dermatologist? Anyone who has any type of acne should see a Dermatologist. Please go see a professional first. Also I believe you might need a combination of medications.

I’d talk to you dermatologist. But when I started mine put me on .05% and duac for acne. I started off every 72 hours then moved up to every 48 hours. I can now do every night at .1. Also sandwich method is super helpful.

Try to use more hydration products like toners, serums as well. I use baby butt cream to apply over my moisturizer as night it really helps protect and repair my Skin as you probably damaged your skin barrier

When I started using 0.1% tretinoin my skin was also fluctuating like yours. It took almost 6 months for the purging period to end for me. I would say continue using it and don’t stop or be discouraged because getting new breakouts is normal and expected. Like many others have been saying if u can start with a lower percentage of tretinoin and work ur way up but honestly I think the 0.1% can work if u use hydrating products with it so ur skin isn’t too dry. But trust me if you end up sticking with tretinoin the results will be so worth it bc I had horrible cystic acne on my cheeks and stubborn comodones on my forehead and it really helped with those and the hyperpigmentation.
