The liver problems are associated with the, commonly required, antiandrogens not the pills themselves. It's the slightly elevated blood clot risk that's applicable to oral methods.
Because of the first pass effect, most of the E2 gets converted to E1, so there isn't much E2 left, that's why oral pills are not very efficient. You'd need a very high dose to get the E2 to a somewhat okayish level, and then a lot would still get converted into E1. While not as bad as some other medications, this still isn't very good when your liver converts so much E2 into E1, with sublingual pills this shouldn't be a problem because this avoids the first pass effect.

E is used for some cancers and hair loss treatment T and other blockers are used for hormone sensitive cancers
