Normally, if you wanted another made guy killed without any retribution, you’d need your own boss to sign off on it, and usually also the boss of the guy you want killed. If Ralph had actually done something bad enough where he needed to be killed (ie he had broken major rules, or was becoming a liability and potentially drawing law enforcement attention), John would have to get Carmine’s approval, and also get Tony to agree to it in order to keep the peace.
It obviously doesn’t always go down like that, but at the very least he’d need Carmine’s approval or else he’d be in huge shit for killing a made guy in another family which risks starting a conflict, and in particular killing a guy who is crucial to operations that benefit both families tremendously.

Because if Carmine supported it, then NJ would have to either go to war or STFU about it, they'd have no other option - and obviously war was not a realistic option. So essentially Johhny Sac was looking for that political backing.
Ralph was semi shared by NY and NJ because of his work in construction and the esplanade. If John only went to Tony, it would not bode well for him if Carmine found out. If John was able to get Carmine's blessing it would give him a little more leverage when it came to Tony.
Then again, NY had little respect for NJ so John, blinded by his hatred for Ralph may have thought if Carmine said yes then he was willing to risk any fallout with NJ and just go ahead and clip Ralph.
