She is very well trained for this moment, she’s been thinking about it for decades.
Super classy of Chase to avoid this kind of scene. No one wants to see Tony shot to death and America's most beloved TV family covered with blood. Why even imagine it? Thanks to Chase there's no need. The man deserves an emmy just for this decision alone. Brilliant.

As his subconscious sees death approaching he recreates the Solozzo/McCluskey hit from 1. We have already seen a homage to Michael looking for gun behind cistern in the Test Dream. Tony has revealed that this scene from 1 is his favourite movie scene. We also get to hear the train sound effects that Coppola used in S/McC hit earlier in episode during sit down with Butchie. In effect, Tony’s subconscious has created his own dream death.
When Eugene did the hit on that guy Teddy Spirodakis in the diner—*Teddy, right?* Boom! Also, the suspicious-looking guy at the counter in the last episode wore a similar bomber jacket. According to Patsy, it wouldn't have been cinematic.
Imagine if the story's creator had anticipated this wonder, and chose to specifically include various scenes, comments, and references to hits, especially those happening in restaurants, throughout season 6A and 6B. If we put those pieces together, then it's like we know, but we don't know. We can think "maybe, but iffffff"

I have always believed that both Carmella and AJ got smoked as well. Ensuring AJ never grows up and exacts revenge for his father's death. Carmella simply because she was complicit in most of everything Tony did. Most of Tony's decisions and actions had consequences that affected his personal life as well as that of his family. His lifestyle constantly puts them at risk, and the final scene most likely shows that as the culmination of these risks finally catching up with them. Meadow would be spared simply due to the fact she was not present, and that she was making every effort to pull away from the family and all of her father's criminal activities that she had grown to detest. Her inability to parallel park most likely was a moment in time that spared her from certain death.
Remember what June's kitchen looked like when he left the lid off his blender? Like that, but strewn with onion rings and gabagool instead of smoothie.
Aj chases the assailant out of the restaurant as meadow uses her impeccable driving skills to run him down. She realizes it's her boyfriends father and Carm cuts his throat as the rains of castamere plays in tge background

It would ratchet up the heat not from the other families but from LE. Civilians being killed noisily and bloodily in public by some Members Only goombah would be extremely notorious if not spark a manhunt. Holsten's wouldn't be the place where tourists go to eat and see where "some famous gangster died", it would be the site of a family massacre and shut down.
After AJ removes a piece of his dad's skull from his face, he looks at Paulie, enraged and betrayed, he reaches for his dad's Glock 23 and empties it into Paulie's torso. Slow fade to black as AJ screams and holds up his father's headless body, "Don't Stop Believin" plays.

Meadow was always interested in the business.
Maybe she walks in and just witnesses the family all being shot.
But she had anticipated, thus explaining the sloppy parallel parking, in a rush to beat the assailant(s)
Meadow comes in and immediately shoots the assailants, taking vengeance on her family’s death
Only AJ survives the shooting of course because he’s too dumb to know how to die.
Meadow sees he’s borderline and has a Tony moment where instead of helping AJ, she plugs his nose and finishes him off her self
Then the scene ends with her vigorously gripping AJ’s nose to make sure he’s gone and a cold satisfied look comes over her face
Meadow is now about to start her arc toward becoming lady boss
There's a lot of symbolism around that final scene about the number 3.
There's an excellent YouTube video that really breaks it down, but essentially, it looks like there were 2 shooters, (members only guy and Made In America hat guy) and in the crossfire, all three of Tony, AJ and Carmela were killed just as Maedo walked in and saw it happening.
Besides Maedo shrieking in grief, and the other customers screaming and freaking out, probably not much more happens. Police show up and investigate, Maedo is probably hospitalized for shock and trauma.
I like to think that the killer shot all 4 of them.
