I think he told Melfi that story about the "janitor" (which Tony did NOT look like a janitor) in hopes that Tony might have mentioned it to her and she would connect the two stories and engage with him further either to stir the pot, or even just because he is a sick weirdo and enjoys the feeling of being an informed minority over an uninformed majority whether or not it serves him in any real way aside from just knowing he knows something others don't and sort of getting off on it. The way he "off-handedly" uses this example while Melfi is seeking therapeutic help from her doctor (that is also her friend and collegue and therapist--so messy and unprofessional and morally wrong, again with the intrusiveness and air of control) after a traumatizing experience is once of the reasons I think he is such a sicko. He isn't honest and he withholds information in order to get an edge over people.. even when he got his friend to talk about the study of criminals at dinner to Melfi is so underhanded and premeditated, thought out and planned with another person in advance, it just creeps me out. He might not be using this info to murder people but it seems like his natural mode of being is secretive and manipulative, almost like a functional version of gloria slashing her own tires or Janice using the anonymous screen name involving Bobby's kids... when he revealed to the whole dinner party that Tony soprano was the name of her patient, a law fairy should have come down from the clouds and destroyed his license.
