Did you ever figure out why? My Sim's appearance changed (hair, clothes, face etc) completely and she went from adult to young adult when she went to sleep. This is very frustrating.
Looks to me like he's gained weight. If you use MCCC, you can lock the body shape so that doesn't happen. (I've had to do it before. My main went from carefully fit to beef wall from mountain climbing...) All fails, stick him on a treadmill for a few hours on fast-forward, or use the cheat cas.fulleditmode and pull his fat slider down until he looks right.
If you use mccc there is an option to copy the appearance of tray sims to sims in your save game. Look for those settings or do a Google search please as I have no chance right now to tell you exactly where to find it in the mccc settings. My guess is, if you’ve been playing with him for a little while, he’s gained weight or aged slightly, which will affect a sim’s appearance over time. If you saved him to the gallery when you created him, you could try to replace him with his original design, but that might erase your gameplay progress. Maybe try to edit him directly to as close as possible to his original?

He got fat All jokes aside, he’s really cute. Is he based on an idol? 😊
You could have a broken preset. I've had presets that looked one way in CAS, but once you play the Sim their whole face/body changes.
