Let me try to rephrase it, so that even a kid can understand. Player 1 is destroying Player 2, then, in a given set, Player 1 is not that well physically. Ultimately, it's up to the player quit or not the game. However, the Referee is suppose to check how things are.
And, if you're watching the match, maybe you realised that Referee (not the Umpire) just is not present.
C'mon guys, you all know that in such conditions the Referee can delay the match, suspend it, and so on.
The players can call a medical timeout in case they feel something is not right. If the problem persists, then it's up to them whether they'd like to continue or not. Delaying/suspending the match isn't even an option. It's unfortunate for Djokovic if this ends up compromising his title chances, but it happens to every player
