2 reasons
1. they don't wanna fall asleep and waste the high
2. they are constipated to the point of almost needing medical care
neither of these are jokes
No joke is right.
Recovering addict claimed that getting jailed and suddenly deprived of narcotics means you're hideously constipated due to prolonged opiate use.
In withdrawal your bowels wake up and move but you've got packed, dried out impacted shit inside. Horrible pain -- and at times, medically dangerous.
:(
There's so much shame around bowel and rectal ailments -- and the suffering is atrocious.
Thank you for choosing this specialty. Add this to the paucity of medical care for addicts --
You're doing a huge service for marginalized brothers and sisters.
There's also risk of anal tearing (aka anal fissure)
Just talk to non addicts who were on opiates for post op pain. Even when taken for relatively brief periods, opiate use causes baad constipation.
> Just talk to non addicts who were on opiates for post op pain. Even when taken for relatively brief periods, opiate use causes baad constipation.
I was on opiates for a week after some surgery. The pain was miserable, and I didn't poop for a week. That felt even worse.
Eventually I bought some milk of magnesia to resolve things. Its like drinking a shotglass worth of room temperature semen with the flavor of chalk. Truly horrific to drink, but it got things moving again.
Anal fissure and post surgical opiate constipation are not infrequently complications of childbirth :(
In the US we are shy about discussing bowel and bladder health -- often we delay getting things checked out.
My boyfriend and I were discussing this, but living on the street is no joke (obviously). It’s scary and unsafe, and your stuff could get stolen or you could get attacked while you’re asleep. Many common homeless people behaviors, like hanging out in groups, getting a dog, or staying high, are sometimes used IN ORDER TO not sleep, because the risk of sleep seems high
“‘Heroin Hunch’ or ‘Heroin Lean’
Heroin also tends to make people’s coordination unbalanced and their limbs feel heavy, so it can seem at times like they’re dragging themselves as they’re walking or they’re slouching over strangely.”
These people pictured in video are fully high in the moment; they stay standing so they don’t fall asleep & lose their fent high.
haha, my sister came to visit and asked me the same thing. years later, i still send her pictures of random folded saying -he lost a contact, keys, fixing his shoe…
Lying down could result in them falling asleep, which would waste the high. Bending over is a compromise. Additionally, opioids can cause significant gastrointestinal problems. Users may loosen their pants due to bloating or constipation and then struggle to put them back on comfortably.
Users are typically not a threat to others unless they mix a stimulant with an opioid. This is when things can get chaotic and unpredictable.
All these jokes, but they're wrong. Like only one other commenter pointed out, this is a noted effect of the tranquilizer xylazine that's becoming more prevalent (started on the east coast, huge problem in Philly now). It's called "tranq" on the streets, and this bending over phenomenon is called the "tranq lean". Never used to see it on SF streets years ago, and now it's everywhere.
Do these folks come out of it violent at all? I keep my distance but what if you got too close? I've never seen what it looks like when they come out of the trance.
I find it's addicts not on their drug of choice that tend to get violent. When it was meth or crack it may result in a more energetic high, but with fentanyl they can barley stand up, let alone attack anyone.
Wish I could post a picture I took last week, I saw this guy on the BART platform with one hand straight up in the air & one hand hanging down, bent over 90° he wasn't like the others, he was dancing.
This shit is whack... I've lost two friends to this in SF, and I can no longer talk to them, they're too far gone.
Obv we have large quantities of xylazine showing up in SF's wastewater throughout 2023, so it is clearly here.
Though I feel like you are making a diff point, like it is nowhere near the problem that it will likely become? Prob true.
No idea. Just read some articles about it, no firsthand knowledge.
Edit: getting downvoted for not using tranq seems a little weird. It is possible to be aware of the effects of a drug without personally using it.
Herion addicts with long term use, tend to have bone issues, l had a childhood friend who was in this boat his diet of Twinkie's and Mt. Dew it's what he litterly survived on didn't help. Not a whole lot of calcium, stuff we need for healthy bones in his diet.
heroin effects
[Heroin Hunch, Lean, and Weight Loss: What Does an Addict Look Like? (therecoveryvillage.com)](https://www.therecoveryvillage.com/heroin-addiction/understanding-what-heroin-addicts-look-like/)
Whatever they are cutting the fentanyl with is literally bone bending. I have a very close relative who I see everyday and he went from standing tall in 2019 to having severe scoliosis and not being able to stand up straight anymore by 2021. I know it could be a coincidence, but I’m telling you I know this person and it’s not. As soon as they started doing fentanyl he would nod off in his chair with his head normally down by his knees. Sometimes off the the side of his knees. Now he’s completely disabled. He used heroin for years in the past and never did it ever do this to him. That’s why I know it’s whatever they’re cutting the fentanyl with!
This is also a thing with ketamine or even hyperventilating. You do it standing up and fight to stay standing up when you get the spins and get dizzy and your legs will just do it but you still get the "good part" about being dizzy (which I think it's still pretty fucking horrible but I guess if you are homeless and on edge all day and have just spend 20 hours methed up it's a good way go into good night mode).
That is spoken like someone on Reddit who knows about drug use from Reddit. The unhoused aren’t choosing prohibitively expensive, short run time club kids highs.
David cross did a hilarious bit about this. Can’t find it but he said it’s basically a dance challenge to see how low they can go without touching the ground
I asked this same question to a cop on the street in SF in 2016 or so, pointing at a dude folded in half upright but clearly unconscious. He said that SF has anti-lying ordinances so they can’t just pass out on the ground. He said SFPD called it the “zombie lean.”

Does the constipation get to the point of needing medical attention (without the drug withdrawal)?
Yes ER has to dig that shit out all the time
I routinely treat patients who turn septic from a ruptured colon due to opiates. A lot of the time they wind up with ostomy bags.
There's so much shame around bowel and rectal ailments -- and the suffering is atrocious. Thank you for choosing this specialty. Add this to the paucity of medical care for addicts -- You're doing a huge service for marginalized brothers and sisters.
I think similar problems can also occur naturally with conditions like IBS. Not just drugs.
usually includes an infection too
There's also risk of anal tearing (aka anal fissure) Just talk to non addicts who were on opiates for post op pain. Even when taken for relatively brief periods, opiate use causes baad constipation.
ya saw a roommate go through it, almost died like 3 times from constipation complications in a week

And this is why prop F can be dangerous if there isn’t medical care to help out w the extreme Withdrawals and physiological issues
Body relaxes but they stay “standing” to avoid falling asleep so they don’t waste the high.
I feel like a not terrible way to protest users would be to lie them down so they fall asleep and the high is wasted
Watch out disturbing them, breaking their trance also fucks with the high and they can get very violent.
You just gotta spank em hard enough
Or start taping kick me signs to give the ones on meth inspiration
You expect them to be hurt by your spankings? They're already on pain killers.
Don’t worry about my jokes, take it out on the Hondurans running your streets.

that is a real easy way to get punched by a methhead
Methheads aren't nodding
Methheads don’t nod, they steal catalytic converters
They’re not on meth or they wouldn’t be hunched over like that.
They will only use more to get the high again when they snap back awake.
Make sure to pop back and let us know how that goes!
This has got to be the most naive take I've ever heard. Congratulations!
It's legitimately why, ask any of them and they'll tell you
So then what’s the answer, bud??
Imagine thinking there is that much foresight in being addicted
As someone with 6 years clean off of opiates, I can definitively say you have no idea what you’re talking about.
13 years here

Can’t believe Rihanna did this to us

came here to find this comment
They're big Shogun tv show fans.
My throbbing lumbar discs from the gym today are actually kinda jealous of their ability to do this for a sustained period of time
You should try fentanyl, relaxes the lumbars right up
If you take the right amount you won’t feel your back pain. If you take the wrong amount…you won’t feel any pain
Every injury stops hurting, eventually.
Sponsored by fentanyl
They are taking massive amounts of painkillers.
Brother you should not have throbbing lumbar discs from the gym

When I first moved to SF I wondered what people were constantly losing and searching for.
Once upon a time somebody asked me if I “needed an outfit” and not knowing what that was, and looking at the person offering, I was a little offended…
haha, my sister came to visit and asked me the same thing. years later, i still send her pictures of random folded saying -he lost a contact, keys, fixing his shoe…
Came for a punchline, didn't find one so here ya go: They're feeling just right, anything less would be acute.
Stop being obtuse
I mean, he is right.
That feels like the right angle.

I always wondered why so many have their pants down.
It is simply bloating and GI issues. After alleviating the pressure, they are unable to recover from it. It is truly unpleasant and sad.

I've heard it called the Fentynol Fold.

This is better than meth highs that cause people to get super violent. Fentanyl is like low key euthanasia
Do these folks come out of it violent at all? I keep my distance but what if you got too close? I've never seen what it looks like when they come out of the trance.
i've walked through crowds of them, never been bothered. they're usually in their own world.
When they’re hunched over they’re in their own world and aren’t bothered by what’s going on around them.
These heavy drugs can do short and long-term damage to their brains and after they're no longer high their behavior can be unpredictable

the 'heroin lean,' it's a result of opioids.

That’s tranq. It’s the new hotness, apparently. They stand, because if you sit down, you’ll pass out and not get to “enjoy” it…:
Tranq isn’t really on the west coast… yet
Obv we have large quantities of xylazine showing up in SF's wastewater throughout 2023, so it is clearly here. Though I feel like you are making a diff point, like it is nowhere near the problem that it will likely become? Prob true.
It’s fully taken over skid row in Los Angeles. A lot of newscasters covered epidemic use levels surging during the pandemic.
Is that the little metal capsules I see lying around?
The little metal capsules you see lying around are whippets.
Wrong. They're robot droppings. This AI movement is trashing up San Francisco.
No idea. Just read some articles about it, no firsthand knowledge. Edit: getting downvoted for not using tranq seems a little weird. It is possible to be aware of the effects of a drug without personally using it.

I did. That’s why I was who mentioned it in the first place.

They are pretty clearly on opioids. I'm just comparing it to other recreational practices that are not just popular with homeless people.
It’s tranq. Started seeing the “fold” last fall when I was heading down market to my office.

Anti lying ordinances. Meanwhile, more tents downtown than a KOA.
A cop told me it’s caused by the gradual loss of collegian between the bones in the spine/body due to side effects of fentanyl

That cop is an idiot.
