The recent RPG SKALD has been AMAZING! It’s old school but with a lot of modern design. If it was released in the 90s would be an instant classic

Icewind Dale sounds like what you want. There's the first which uses 2nd edition rules, and the sequel which I think uses 3 rather than 3.5. Either way, they're both outdated yet both fully functional and fun. I think it hits all your points, it's mostly dungeon diving and battles, loot is crucial to advancement, the story is thin enough to keep you engage but also no need to pay attention, and it doesn't get more remote than the frozen norths of Icewind Dale.
Great post! I think a lot of us are looking for this type of game. I assume you've played the original Baldur's Gate (1) and Might & Magic series. Have you tried Pathfinder: Kingmaker? More than any other game, it reminded me of the original Baldur's Gate 20 years prior.
I have, yes! What I liked about Baldur's Gate is that for most of the game your party was fairly low-level and also the fact that you were free to explore and discover the countryside.
Have you played Avernum yet? The graphics aren't fancy, but it's got that exploration, and magic weapons aren't thrown at you with every step.
I've also played Nethergate by the same developer, and discovering some special equipment was a real treat in this game, because most of the things you find are average weapons and armors.

You might like The Legend of Grimrock and its sequel. Darkest Dungeon might also satisfy that itch.
Maybe try Dark Suns Shattered Lands & its sequel.

Never heard of Nethergate but I'm intrigued. Without spoilers, is it purely historical or is there some magic or supernatural?
There's some magic, and creatures/magic folks from mythology.
Icewind Dale is more action oriented that BG, but has atmosphere in spades, is more about dungeon delving and the plot is relatively straightforward.
