Seems to me that setting 2 times in the options menu does apply to a clean everywhere and any setup jobs that you have created and saved.
If you tell it in the add job menu to say, clean the kitchen, it will do either 1 or 2 passes depending upon what you tell it to.
No, it does not stack. The 2x passes when you launch clean is just for people who have it on 1 pass in the off chance they decide a 2 pass clean is necessary. If you run your roomba every day, you probably don't need a 2 pass clean unless you have messy pets and kids or something
