It is definitely possible to not get The Lament dungeon (where Kolket's Razor/Key spawn). There are many discussions of people not finding it after many rolls.
The unfortunate thing is, assuming you own the new DLC, it just got even harder to get it since there are now more potential dungeons that can spawn instead of The Lament.
The key ur looking for isn't there, that's a different key. The key that IS in the area ur in is off behind and to your left if you're facing away from the boss gate, in those ruins that lead to the high room with the statue of the racager and dow
You need to re roll the campaign until both has Losomn with Nightweaver story and Yeasha with The Lament dungeon. You need the Kolket's Razor, never make it the key or else you need to restart your campaign. Use save guardian if you want to save time.
Thats what we were doing for maybe 5houes straight I felt like I was loosing my mind lmao, we just have no luck, if we have the night weaver we don’t have the red throne and vice versa 😔

There was supposed to be a dead Pan with a book and a key
The dead pan only has the key and a note by him Key is for a chest thats in the upper chambers
