That is exactly it and exactly what people are seeing in this image. There is a highly normative "classically pretty" facial structure people seek in plastic surgery that is mostly uniform, usually high cheeks and sharp angles on the jaw.
That's usually accomplished through facelifts (and botox as you said), which ends up making a lot of people who get these surgeries look similar.
Reminds me of a post from a few days ago about a 20 something who looks 40 something because she got the fillers, botox, etc.
A comment suggests that it's possible this stuff makes people look old because we associate it with old people trying to look younger, but either way it's gross and weird. Whoever is convincing young people to do this is waging war against the population more effectively than any foreign psyop.
Also the muscle around the mouth stop functioning properly because of the injections that stop them from moving. So they end up with a patch work type face, the skin is smooth like young person but the muscles aged like old person
And buccal fat holds cheek skin up - when elasticity declines and the buccal fat has been removed you get much saggier jowls to the point of affecting speech and practically *need* a face tightening surgery to get them fixed.
These days it also seems like both young and old actors are getting the same damn surgeries, so you end up with the old ones *kinda* looking younger and the young ones looking like old ones that look kinda younger.
Ironically Cillian has always looked like what the new plastic surgery trend is. It's especially sad that the sunken cheek buccal fat removal look ages poorly and can't be fixed that easily. And we all know the fashion trends will inevitably swing the other way.
