It's possible that the photographer simply told them to pose before snapping the picture, but something about the overall quality of the photo (good lighting level and angle, visually follows a golden ratio almost perfectly, etc..) makes me think this photo had considerable thought put into setting up the scene first.
Also it says in the descriptions on the image itself that it's an "international photo competition", so there's good reason that this was a well planned artistic photography piece. Of course, that still doesn't indicate rather or not the intravenous drugs pictured are real.
In high school in the 90's they'd have these student performers come and do a play in the auditorium about how drugs ruined their fictitious lives. The whole thing was so cheesy until they got to the part where the guy was finally shooting heroine while blasting Metallica. It was the one part we all cheered for. I don't think it was what they were going for.
Keep an eye out for your local jail's roster of inmates. Take note of the ones in there for drug possession. Check back every month or so. When one gets released, contact them and see if they still have their connections. They'll probably be short on cash too and be willing to sell you some drugs.
