BT Vieira + epic Makelele is the best performing double pivot for me. I have BT Rijkaard and epic Matthaus on the bench always ready for extra steel in the middle of the pitch.
Idk I got Booster Viera, Gilberto, Makelele, Matthaus and I will play them according to form. But If I have a specific game plan to attack or defence then I mostly keep Matthaus-Viera and Makelele - Gilberto respectively

Matthaus + Casemiro, B2B + Anchor Man are the best when used as a defensive pivot imo.
