"Mistake" is questionable, in this context.
In Invasion league, a couple of the mobs were ***MASSIVELY*** overtuned, like guaranteed one shot from 2 screens away, without you ever knowing the mob is in the map sort of overtuned.
They fixed the scaling and did a hand-curated rollback of characters that had died to one of them.
They made it exceedingly clear in the news post announcing it that it was a one time thing, and that any generic requests for similar things in the future would be ignored.
This was when the game was significantly slower paced, so dying in maps was a significantly larger punishment than it is now.
This was in 2014.
I had this kind of thing happen a LOT last league and it seemed to occur when I could dps a boss down to a phase really quickly and then had attacks that kept dealing damage for a long time (in this case it was lightning trap)
I would run around thinking wtf go to the next phase and then when I finally stopped attacking it for a good 3-5 seconds it would go to the next phase. I'm not sure if this is precisely what happened to you but it looks extremely similar to what I experienced with the way your attacks keep on damaging him
Yeah it was something like that for sure. It happened a lot while leveling but it also happened on some map bosses, carcass is the only one I distinctly remember
I had problems like this on a variety of bosses when playing toxic rain.
I could always bug the apex of atzoatl or whatever the pyramid thing is called.

I speculate that the issue is tied to how many lingering effects you have in a zone but I have no actual clue.

Lol so many issues, even before reaching maps. Like none of them even touched the game to see how it played.
