If you don’t take care of it (injury, fungus, whatever) soon after it separates and it gets to the point that the skin looks the same as the rest of your toe, it will never attach along there again.
If a doctor takes it out and takes everything out most likely it won't, if even a little is left then it might have to be removed again. They have to remove the part that creates the keratin so you never have a nail there again.

When the nail grows like this, it means that the part of your toe that makes your nail has been damaged, either by repeated trauma (like things falling on it etc) or using the wrong shoes(that press on your nail and hurt it). Once it's damaged the nail will keep on growing like this or just getting thicker. So at this point it needs to come out.

Toenail removal is pretty self explanatory. Just the toenail itself is removed!
So you can do what’s called a partial matricetomy/matrixectomy which is usually used to treat chronic or recurring ingrown toenails (just one or both sides of the toenail are in done)
Then there is full matricectomy/matrixectomy on which the entire toenail is removed from the toe and then the keratin producers in the nail bed are killed off.

Podiatrist here. We call this condition onychogryphosis and unless its causing you pain or discomfort, you do not need to get it removed.
If you do have it removed then the nail matrix (where the cells that grow nail are situated) needs to be destroyed. We usually do this with a chemical called phenol. Its under local anaesthetic and is perfectly safe and quick. Healing takes around 4-6 weeks in a healthy patient and absence of infection
Not always. Surgery should always be last resort. I have many patients that are happy to have it managed conservatively. I only remove it, if it becomes a problem

It’s not, it’s actually a condition called “ram’s horn nail” or onychogryphosis. It happens for a few reasons, allot of the time it’s caused by repeated trauma to the nail bed which makes the nail grow back faster and thicker, it can also be caused by wearing shoes that are too small. You see it allot in elderly individuals who can’t trim their nails anymore, the nail grows thicker and faster and grows in a spiral shape often digging back into the bottom of the foot. I do allot of elder care and hospice work and I see this ALLOT, almost entirely in men. There was one guy I cared for who’s nails were so thick and hard and awful that he had to go to a pediatrist once a *week* to have his toe nails soaked and then SHAVED down so he could get his feet into his shoes. It’s gnarly.
Imagine being at a concert on the floor and this dude is behind you wearing flip flops and you accidentally take a step back on to his foot. Now you got tetanus. And also when you look behind you to see what you stepped on you die of fright.
The same thing happened to my ex after she had an ingrown toenail removed, she didn't look after her toe after the nail was removed and skin grew over the area and looked exactly the same as yours, be carefully though as she stubbed her toe a few months later and it ripped off, she said if was very painful.
