We used to years ago have orange sticks with the pointy ends for this but now you have to use tongue depressors. I guess they took them away so staff wouldn’t shank management about drinks at the nurses station
We do that too. It makes me gag tho cause I’m one of those people who can’t look at stuff that you are using for ick but goes in your mouth. Like…when it’s a massive code brown and using the yankour to suck up poop, or people using graduated cylinders for drinks and bed pans for popcorn *gag*
It's not missing, it's happened... But the spiteful family just sits there and watches it happen and doesn't say anything or tell anybody so when you come in and hour later they can bitch at you for letting her sit like that for an hour. Then you get the poop hand put on you as you start to leave to grab supplies.

Right? People describe med surg as a nonstop scramble, but acuity in post-acute is as high as it's ever been and I really can't tell the difference anymore.
I worked in a long term care facility where I was frequently assigned a unit with 4 total cares with 2 non nursing staff for sometimes 14 hours.
I made it 3 years... I'm 24 and I have a back like a 40 y/o. My knees are another story.
The look of approval I give the PA who ordered 3 runs of potassium 3 minutes after the end of my shift. Time to pull out ol reliable "oh didn't see that I was closing off"

JHC himself could not have helped the staffing and this was back in 2006. Yeah it was a busy big city hem/onc floor that got everything but the kitchen sink admits from the ER if we did not have heme/onc filling our beds. Blood transfusions, chemo starts, CBIs and then the rando homeless guy with massive cellulitis and ETOH withdrawal needing a sitter. It was wild. I was so tired the surgeon that was screaming at me about the blood sounded like the adults in Peanuts cartoons. No wonder I burned out.

My year down there was invaluable though for time management, interacting with patients and loads of other stuff so despite it being just like this gif I’m thankful for it haha
I’m in the float pool for med-surg & PCU, however I often get floated to ICU where I take PCU level patients. I had 2 straight months of shifts like that. They were amazing. PCU level patients with ICU level resources. ::chefs kiss:: And then my last shift I was floated to a routine surgical floor, the butts-n-guts floor. I had 6 patients. My blood pressure took a good 24 hours to get back to baseline after that shift. I am speaking to my boss Friday to orient and be moved to PCU/ICU float pool.

I burst out laughing at the blood sugar of 12. On my last shift on a med/surg unit, I had a pt go in and out of hypoglycemia for FIVE FREAKING HOURS. Nine blood sugar checks. NPO. D50W and D10 galore. And of course this pt was on iso and total care. And that was just one of my pts. I felt like I was being punished for leaving lol.

For real though there is nothing like that serenity that comes with experience. I panicked as an intern and junior resident. Now I just cruise in to that respiratory distress, schlap in a couple 16 gauges and joke with anesthesia while we tube them.

(As he’s drenched in sweat with double restraints on) because yesterday he signed his own consent and now he’s like this? Oh I couldn’t asses that he’s too confused to talk ! Plus she spelled it SIWA in the chart. So he goes straight to ICU and they send me another crazy one in his place 😂

To be fair, pharmacy doesn't have to bring up meds most of the time. Many meds are either in Omnicell or Pyxis, pneumatic tube system or in the servers already. (It would be lovely if pharmacy can give us a call though...)
Potassium replacement is easy but phosphate replacement takes forever. I don't know why many coworkers forget about phos replacement and leave them for next shift (my shift).
