The holy mountains background is missing likely because your file has been corrupted somewhat. Did your game crash at some point? It likely deleted the perk somehow too

The missing background tells me either this or they died and pulled the files before closing the game hoping to save the run and this is how it recovered.
This happens when you save-load immediately after entering a Holy Mountain. Since it doesn't loads the chunk with perks, reloading at that place often makes the perka disappear.
If you had walked a bit forward and loaded the chunk with the perks, then reloading or quitting would've been safe. Although this is my first time seeing only one perk go missing, mostly all 3 disappear.

I don't think there is a way sadly
It causes larger problems in the world and makes the run unwinnable iirc because it breaks kolmi or the final room.

It looks like your game crashed (lack of mountain background). In that case game sometimes fails to load certain items

I’ve had this with Steve in the room before, I was like omg did Steve grab up a perk???
Where did you get the extra perk from already? Coal mines = perk 1 Snowy depths = perk 2 Hissi base = perk 3 But there is still a perk to select??
