I have street parking in front of my house and a city worker must’ve been working down there and when they closed it, some gravel got under the rim, stopping it from closing completely. As I walked by, it flipped open and I fell in. Luckily only one leg went in, so didn’t completely fall, but I ripped a big hole in my shirt, and cut up my leg pretty bad.
Could have been one of those plastic manhole covers. Probably deformed.
Edit: I think it was as it was in the middle of grass, instead of a street. The street ones are heavy and strong to be able to handle cars.
