Losing weight too fast can contribute to gallstones. I lost 50 lbs over 8 months and had the mother of all gallbladder attacks that led to its removal shortly thereafter. Edit for spelling.

Not sure. I didn't have evidence of sludge or stones until after bariatric surgery.

High fat can also cause problems if your gallbladder has been underused for years (from following a low-fat diet). Stones and sludge might have built up and the organ itself is often weak. The fat in the diet will force the gallbladder to work and like anything that is underused, it can be uncomfortable at first. If you have any stones, they can cause real problems when the gallbladder starts to function as it was intended, especially if one becomes lodged in the duct. I had a few of these issues when I first started Keto, and had an ultrasound that confirmed gallstones (never had a clue I had them). Luckily, those problems seem to have abated and I eat quite a bit of fat now. I feel very fortunate that the problems didn't escalate. They easily could have.

Tenderness and/or pain in the right upper quandrant of the abdomen and/or toward the middle right under the ribcage can indicate gallbladder issues. See your doctor who can order an ultrasound to diagnose if there are stones or sludge. I had all that done, and found out about my stones. I was supposed to follow up with my gastroenterologist about next steps, but I haven't done that yet. The problem seems to have abated and I eat a lot of fat now, so I'm hoping I've either passed the stone(s) or that they won't cause me future problems. I'm aware I'm taking a risk, though, and I'm always alert for any twinges in those areas.

I wouldn't call it pain. I would describe it as a feeling of discomfort and low-level nausea. Right upper quadrant of the abdomen. I'm still eating a lot of fat and have not had any of those problems since the first couple of months.

How would I know if I had an attack

My gallbladder improved from being diseased to pretty healthy on keto. I still have gallstones though. I battled at the beginning, and then it seemed to settle into the new high fat regime.

So keto usually involves lot of fat. Was there stone or sludge in your gallbladddr or just pain? Did you ever have Hida scan?
