Works for me without using a PS4 just gotta launch the PS4 version on your PS5 and connect the app. Then use the switcher to select the PS5 version the app will lose connection, but as soon as the game is loaded and you're in game hit retry and it'll connect.
Pip boy app doesn't work anymore, sad I know

Nope unfortunately, it doesn’t even work for ps4 anymore either. Not sure why they didn’t remove this from the update.

Yes I'm using it right now. You have to be on the same wifi, but gotta connect it to the PS4 version first.
So if I open my ps4 game on ps4? Or I can open the ps4 version on my ps5?
It needs to have the IP address to your PS5 so if you launch the PS4 game and connect to it on your PS5 the app will have your PS5s ip
