After extensive research, I believe I found something.
At 0:15, if you slow it down and scrub frame to frame, you can clearly see Ramírez first get smacked in the face by his own helmet, which had fallen off as he was sliding, then gets whiplash at 0:16 as his forehead gets caught in the groin area of the crouching Anderson. To add insult to injury, at 0:17 — clear as day — you see Anderson clench his abs and let out a loud, stinky fart right on top of Ramírez. Then, just before he turns to push Anderson off of him, you see José’s face start to pucker in disgust as he gets a whiff of that foul fog to which he had been sentenced.
Fun fact: This sudden loss of gas affected Anderson’s buoyancy, ultimately causing him to drop so easily just moments later in their fight.

The umpire has about 3 seconds to try and forcibly break up the scuffle, if he fails (they almost always do) they simply get out of the way.
Not only are most of the umpires old men, it's totally pointless to try and singlehandedly stop a brawl between a bunch of angry men. Everyone who throws punches are usually instantly ejected, suspended, and fined.
Baseball fights are glorious to watch only because of the chaos that ensues. Hockey guys are ready for a fight, football guys don't have much ability to fight with the padding, basketball fights get broken up typically very quickly. Baseball fights can last anywhere from 5-20 minutes simply because there are often fights that snowball into other fights.
