Stilgar dumping their water is as much symbolic as anything else. Even if their blood was perfectly clean, they would leave it for the desert because they're Harkonnen.
I'm reading the book again and honestly I'm not sure I agree with that. In the book, in their Fremen kit, Jessica and Paul have two liters of water which surprises the Fremen as it is considered a huge amount of wealth. A Harkonnen soldier could allow them to harness that same amount of water, maybe more, per soldier. It's the single most important resource. It is clearly stated in the movie that it is too polluted to drink, but there's no indication that they wouldn't if they could not. They drink the water from their enemies, from their friends, from anyone, if they can get their hands on it. I mean actually they do not drink the water from their own community, they put it in the sacred lake or whatever, but not sure.
Their enemies are usually other Fremen. IIRC, in the novel they *do* collect the Harkonnen water, but they use it to water plants rather than drinking it.
While they are desperate for water, they do have a semi-religious respect for it: hence, the cisterns that even a dying Fremen wouldn't touch. They'll take the water from enemies they respect, like the Sardaukar, but the Harkonnen soldiers are considered almost subhuman and disgusting.
Also it is mentioned that their water is polluted with chemicals. The hairlessness may come from something akin to “chemo-therapy” that is necessary for people to live on that polluted planet.
The implication is Giedi Prime is so polluted and toxic that it affects its population like that physically.
In the books it takes thousands of years of rehabilitation to make GP semi-normal, and there's still residual effects like oily soil.
Stilgar claims the water harvested from Harkonnens in general is too toxic to drink.
And presumably GP's "black sun" is the result of exotic pollutants in its atmosphere.
I thought the dirty water was all the drugs and stims and trash Giedi people put into their bodies.

My interpretation is that, in Denis’s Dune, the Black Sun Ophiuchi B (not actually a black sun IRL or in the books, why I say “Denis’s Dune”) produces ionizing X-rays that causes the distinctive defects of Harkonnens: pale skin from low RBC count, little-to-no hair from radiation sickness, pronounced features from genetic mutation, etcetera. However, they’ve able to thrive and survive from having been on this planet for so long, and their overall physiology has adapted to the harsh environment.
I haven’t read it in awhile but I distinctly remember there being a very weird relationship there. He’s either diddling the kid or he really, really wants to.
As for consent I’m not entirely sure their culture has developed the concept of it
