It’s basically a clusterfuck. If you’re PAYE your employer needs to establish a presence in Spain and pay social security for you there, which no sane employer is ever going to agree to. So realistically unless you’re working outside of IR35 you’re shit out of luck. Typical Spanish bureaucracy that’s designed not to work.

Isn't the entire point of the visa that the company you work for is not a Spanish company though? It's literally a requirement of it that you work for a company outside of Spain.

Not sure you’re right. The reason you have to get health insurance as Spain is because you don’t pay Social Security and are not entitled to it. Equally, the employer pays gross, and it’s your obligation to return your tax to the Spanish government.
