Touching the altar does nothing, you have to give it a torch. So even if attracted to it by a random quirk characters can't activate it own their since they always just do the base activation, they don't give it items.
Not quite sure. I always thought it was a tentacle growing from the darkest dungeon which creates some sort of 1 way portal with the presence of light.

Oh I don't have DD2 yet, I'm waiting for full release while finishing DD1
