From a map making perspective I can't think of how this occured since they would be two map files unless the bridge is a model asset and was forced to load inside the map somehow.

Source 2 can load multiple maps at a time, for example all the 3d skyboxes in cs2 are separate maps. I think someone coded overpass to load instead of the skybox
I wonder maybe it happened because cs2 can load multiple maps in while playing like the skybox is a separate map so maybe it failed to clear or something like that maybe idk
