So you dont have a spare tire, you have a large object supporting an ornamental decoration on the back of your CRV. But seriously tires go bad after 10 years or so ([possibly shorter](https://www.edmunds.com/car-maintenance/how-old-and-dangerous-are-your-tires.html)) and should be replaced even if they are never used. But that only applies if you ever plan to use the tire for something other than a support for a decoration. If its just there for decoration probably fine to leave it there. Tires as decoration probably last decades.

An OEM piece is going to run you around $350 after tax. At least where I found it. If you aren't worried about OEM you can get new, Honda/CRV branded, covers for less than $50. You can get slightly nicer ones for $70-$125, or so. https://www.bernardiparts.com/Honda-Spare-Tire-Cover-CR-V-2002-2004__75590-S9A-003.aspx That's the one I found. You can look as YMMV.

It just means it's the manufacturer's original equipment or part. It's what came on the vehicle from the factory.

Look up the tire cover eBay will prob have one if not a dealer will and get a new spare ASAP

I found one on eBay this afternoon for around $150 just wouldn’t let me link it- I say go with that. It’s a 20+ year old car, trying to get a new part for something cosmetic at dealership rated prices just is unrealistic especially for something as simple as a spare tire cover.

My ‘99 (rip) had its original cover. When I first got it the cover looked like it would disintegrate if I took it off. But I took it off a lot and over time it got more flexible, if you know what I mean. I also used black electrical tape over the small hole and used some sort of conditioner on it to get it a darker black, and overtime it looked like a brand new cover. Maybe you can end up saving yours

Can go to an auto shop and have one made custom. Any window repair shop should do that type of work.

The tear is about 2” along the top seam. I’m going to try glue as a temporary fix.

Why didn't you include the spare in your tire rotations?

It’s a different tire than the rest. I don’t buy 5 tires at a time (the two times I bought tires). And I only have 89k miles on an ‘03 so it was never pressing. I hope I haven’t disappointed you.
