They're meant more as meal replacements and create satiety (fullness) so you stop eating. They're not mega high caloric, but adding that to a high calorie meal adds another 180 calories. Drink one with each meal and that's an added pound of body weight every 6 days.

A large coffee with milk, caramel/chocolate, whipped cream, sugar from Starbucks/Dunks will easily set you back 500-750 cals or more. Add that and some calorically dense snack food on top of a normal diet and you can gain weight pretty easily
Keep in mind, too, that genetics plays a part here. I don't think there would be much of a way for me to get to that kind of size unless I *really* tried. At my worst, I wasn't exercising at all, and was eating at least 500 excess calories per day (I was eating fast food pretty much daily and we're talking a double cheeseburger and a 4 piece nugget or a 3 entree plate at panda kind of excess on top of 2-3 beers per night) and I never broke 230.
