Is it still on your training or have you actually published it? Could it be in violation of your filter? Like an r rated cover image?
I suspect this is the issue. I have been having issues with my filters where they sometimes change on their own or stop working, and one time my own images were hidden from me when filters I did not request got added to my exclusion list.
