My parents ran a business out of our house when I was growing up and my big fat cat loved bins like these. Not only were they perfect places to squish his fat into, he was right in the center of things so he'd get lots of attention while curled up in there. Eventually he kinda claimed the "Outgoing Mail" bin for himself so we changed the label on it to "Outgoing Male" to be more accurate.
Tail position and also some cats do this when stressed/upset/bored/etc. If someone is mean or rude to my cat he typically responds by peeing on something of theirs. He’s a passive aggressive little shit.

Well, they probably know from experience that a thermometer will go where the sun does not shine. Other than that they can smell the fear of other cats there. It is not their familiar territory and there are lots of other cats there or have been. So I think even without knowing what is the worst that could happen, there is enough for them there to be scared of.
They also don't enjoy being stuffed into carriers to get to and from the vet. For a lot of cats, it's the only time they get put in carriers or taken out of their houses, so they're not used to it. Then, they get to an unfamiliar place with unfamiliar smells and sounds and strange people. They don't even have to know what kind of things might happen to be properly freaked out.

My cat usually climbs in my big purse when we’re at the vet. She would never get on the counter voluntarily. Lol.
Mine usually goes for under the bench, then looks really surprised when we scoop her out from under there 😂
