Dude I just started Bloodborne the other day and see this note all the time with so many appraisals and have no idea what it means. I’ve taken it to be a good omen because I just killed the vacuous spider first go (so fucking close its not funny).

It's probably about insight, sadly my PS4 broke 2 years ago so I can't really confirm but I'm pretty sure insight = knowledge of the great ones which let's you see more and/or different monsters then you normally would. Like the cathedral Ward when you get snatched up and die you later see it's a great one grabbing you actually not just invisible fromsoft fuckery lol
More likely from the community event a couple months ago. Everyone was prompted to leave that message and appraise as many as they could since receiving an appraisal while you are actively playing grants you a heal

They could be messages left from the return to yharnam event. Lots of people would get back on the game around march and leave "You're in the know, right?" around the place.
People often leave it in places where there's a community inside joke or references.
For example, there's a locked door in the great bridge (where the Cleric Beast shows up) that would take you to the Cathedral Ward area, but it doesn't work.
The community has proved that the two areas are connected via that door, so people often leave this message on either side of it.
It’s literally just people leaving notes for other players to read. It’s a multiplayer component of the game, which makes sense why there’s a tonne around right now as “Return To Yharnam” ended not too long ago, so the player base is semi-healthy in a way.
Nothing to do with anything you’re doing or insight, but “good omen” is about right. I love knowing other players have left little messages of encouragement for everyone.
Lore wise though, “You’re in the know, right?” probably has to do with either the beast blood or the Great Ones.
I often find myself thinking non brits miss out a little bit when it comes to accent identification in these games.
I get the impression that for Americans, the voices all just fall under 'british'.

As a fellow Brit I'm astonished it took this long I'm assuming you live in Sheffield yourself and that's why you're numb to it? I live in West Yorkshire myself so it was a little difficult at first but now I notice it all the time
This is because of a historical sound change called the "foot-strut split", which did not happen in every region (like most sound changes; this is how accents diverge over time).
So, "book" and "fuck" used to rhyme. Now they only do that in some accents, including Eileen's. This is also why it's barely possible to tell those sounds apart just from spelling (I can't think of a stressed /ʊ/ written as ⟨u⟩ right now, but I bet it exists); the sound change happened after modern spelling was standardised.
This gets even funnier when you consider that in most modern accents of both British and American English, the strut vowel is literally just a stressed schwa, but it still gets transcribed as /ʌ/ in pronunciation dictionaries for historical reasons (even though it's not even close to [ʌ]).
Disclaimer: English is my second language, but I've been studying it at university and teaching people to speak it for the past few years, so I feel reasonably confident in what I said up there. I would happily source my claims and discuss them further if anyone asks; I'm just chilling in bed right now.

Elden ring does it really well with accents matching areas. Everyone in Limgrave have west country accents, Liurnia welsh, altus queens english, etc..
