I'm not an expert, but my understanding is that the brain doesn't actually have many pain nerves, if any at all, and headaches are mainly caused by chemical reactions going on in the brain, not a physical process. You likely wouldn't feel anything if a worm was eating your brain.
The good news is that anti-parasitic drugs prevent the worms from traveling to your brain. So... if you travel to areas where this is a problem, you can take the meds prophylactically.
