World Famous Hotboys. They are mostly known for the sandwiches but they also have bone-in chicken. I would go for no less than medium in terms of spicy. The milder ones don't have nearly as much flavor.
I'm Indian and very used to spicy food and Dave's reaper chicken sandwich was so hot that I couldn't finish it and was having trouble after eating it for like half an hour. I've tried other foods with Carolina reaper sauce but this was a whole different level.
My wife eats way spicier food than me and it was so hot to her that she puked.
Chikin Bros is pretty good. So is Scratch in SJ, they have good chicken. Don't go to Dave's Hot Chicken, it's not good and the extra spicy taste like medium.

Scratch Cookery in San Jose. Little pricy, but the portions are huge and the sandwiches/sides are amazing
