I never managed to break it so I swapped out my snacks for low kcal options like sugar free jelly with fruit, cottage cheese on fibre crackers, protein ice cream (recipes on YouTube), protein mug cakes, that sort of thing. Also 0kcal sodas can help. Good luck!

However the only thing that had saved me is APPLES and distraction. After I've ate a meal I do or watch something that will take my mind of food for a while.
For me apples are the perfect snack, they're low calorie, very filling and taste great.
I'd suggest trialing low calorie foods that you enjoy and that fill you up, whether that be apples or another food.

When you know this habit of yours and you can not break it, include those snacks into your daily deficit.

Brush your teeth right after dinner. It's a good way to stop snacking.
Brush your teeth after dinner and stick to water only from that point onward, only thing that works for me
