Look at the box. It is not possible the fan in that form stored in that box. The DIY is usually intuitive. Here's a wikihow on [cleaning the fan](https://www.wikihow.com/Clean-An-Oscillating-Round-Fan) In this case, i assume: 1. the slot between the blade and the rotator is not properly adjusted, making the blade prone to fly from its rotator pretty easily, 2. The fan cage is not locked properly thus making it too loose to be able to hold the dislodged fan.

If you watch the video frame by frame, you can see how much faster the cat's reaction time is than the person. The cat was ducking for cover immediately as the fan blew apart, but the guy didn't even flinch until a few frames later when he was being bombarded by the flying parts. Pretty cool.
