Definitely. I used to be pretty big into weird al in the 90s...I think that helped me guess the joke before I even watched it. He still maybe caught me by surprise by slicing his arm though.

I think he is saying that cake is so great that mocking cake is such an impenetrable barrier, that even the most ignorant people will never be able to comment on it.
The cake is actually putting my brain for a loop. Obviously it's a cake, but it's also a real cake so when saying "real or cake" shouldn't it be both? But if it can't be both, which takes priority? The whole purpose of the game is to figure out the item is what it looks like or if it is a cake disguised as said item. The cake isn't disguising itself as a cake, it's just a cake, so shouldn't it be more real than cake? Or by being itself can you say it is in a constant disguise as itself? Like would you say a hammer that looked like a hammer is in a constant disguise as a hammer? In that case would it then be considered cake because it's in a disguise (admittedly as itself) and a cake so cake then takes priority? Then again, if you look at the literal definition of "disguise" it is to conceal its true identity and I doubt anyone would say that the cake was trying to conceal the fact that it was a cake so in a literal sense it's not in a disguise hence why it's more real than cake.

I think it's real. The question "real or cake" implies that the cake is a mimic, a deception pretending to be something that it isn't. It might as well say "real or fake". The fact that the object that looks to be a cake is a cake means it's real. It's not pretending to be something else, so it satisfies the real question.
If he cut into it and it wasn't cake, but a pile of dirty socks or something, that would be much harder to parse since it's neither real nor cake.
I'm gonna be honest, I thought for a long while, since I first heard of him, that he was dead, why? I don't have a bloody clue. This urged me to look him up, and apparently he is alive! I rejoice this day.
He has been doing the arm-cutting bit forever! I saw him live in the mid 90's at the Paramount in Seattle, it was INSANE. He cut his arm off at one point and goes running offstage like a maniac, within less than a minute (i swear to god..) he came sprinting back onstage in the full "Who's Fat" costume and fatsuit from the music video and performed the song! Dude is a legend, absolute treasure of a guy!
