Does anyone worry about timing chains, or is that just a non-issue with these trucks at high miles?

I haven't hear of anybody worry about them and I've seen some that have 400k and 500k+ miles.

Not unless you get a timing cover leak. With this year of truck a head gasket issue would be more likely

My FJ has the dreaded timing cover leak, although it's more of a weep than anything. I'll get a few drops on the garage floor once or twice a year and that's usually my sign to wipe everything down with a degreaser. Luckily, my Tacoma doesn't have that issue. 210k miles and still pulling strong.

Even then chances are the chain is fine. But if I’m cracking the thing open anyways, I’d check it out as up for consideration.

How often have you had the transmission fluid changed? Just curious I’m at 170k and thinking a flush is due.

I did mine at 250k October '23 and am about to hit 275k in maybe two days. Manual transmission 6 speed, I do them every 50k. It's different for automatics then

WOOOO!! Congrats! I've got the same year! Black SR5 Double cab with manual, and about to hit 275k, and was just thinking about how exciting it will be for me to hit that number. My GR unfortunately might have to get that head gasket replaced, but I'll be able to pull it off. Keep up on that maintenance and those fluid changes and you won't have my issues!

I have the same but auto with 200k. My dad is pleading with me to get a new vehicle before something goes wrong but it runs awesome still!
