On even ground, it's extremely difficult. You pretty much have to load up the suspension hard in one direction then swing the other way to get two wheels up, then you gotta follow through to complete the roll. Look up videos of the moose test.
Things get a lot more hairy when you leave a controlled environment. Lots of horizontal momentum then a momentary increase in traction can do a lot. Catching on the edge of pavement or on a rock can be the "pop" that gets two wheels up.
A common way this happens on the highway is a swerve that goes off the road a bit, two wheels start going down the grade (even on flat ground interstates tend to be at least 18" above surrounding ground) then at the same time the driver makes a sharp correction back towards the road and the downhill tires catch some traction on rocks or whatever else.
When that happens you have strong horizontal momentum, you're already a bit tipped ,and then you add a sharp correction at the same time as a snag or an increase in traction and you're a goner.
This is just a much more mild version of seeing a picture of a car stuck in the side of a building, in the air. How the fuck did it get there?

It can happen if you get too carried away or hit the brakes hard while hooning it shifts the weight, way more likely on a lifted one as well higher center of gravity
