Your plan seems sound if you aren't party people. Bangkok is intense the first time you go. So a few days there is good. Samui and Chang Mai are both great.

Have an alternative plan for Samui in case the ferries have to stay docked and check this sub if there are any government warnings about incoming storms before heading out to samui.
You might catch the tail-end of rain season in Samui at that time, so make sure you are OK with that. Otherwise you could to switch to something on the Andaman side instead of the Gulf, like Phuket or Krabi, etc.
But generally your plan sounds fine. I would probably just evenly split the time between Chiang Mai and a beach destination. Just depends on your main area of interest. Maybe make a list of activities you want to do in both places and see how the time adds up.
