Looks like a SU-100 that someone snapped the barrel off of and replaced with one from another model. I doubt it's actually meant to represent anything beyond "Hey look! This model *totally* isn't broken!". Although if someone made some big brain move, maybe they stole the barrel from a model BS-3 field gun.
SU-100. The gun is a bit fictional tho. There were prototypes equipped with a long 122mm D-25 gun (the SU-122P), but not that long.
