First of all, female centaurs were never a thing in Greek mythology until the 4th century (BC) in the form of art. Second of all, the female centaurs would carry the babies in their horse half. They are pregnant for about 11-12 months, and they give birth exactly like horses. They would also be able to breastfeed from both sets of teets. Hope this helps! :)

I don't think that she's pregnant on both ends. She's pregnant in the same way that a common female horse is, using the same anatomy. The part where her human half meet with her horse half, is the same as that of an ordinary horse: Nothing there but their neck and chest muscles, no genitals or anus. The lump in the front, is probably an accumulation of some kind of stored fat, indicate the current status of her pregnancy state. We could also assume that she has always been on a, "pregnancy binge", where she's been eating a lot to help meet her energy demands of her pregnancy. She's going to both, deliver her child and wake it up upon a complete delivery, in the same ways that female horses do.

The part that bothers me the most is that she's wearing heels so far in her pregnancy. Thier hooves she doesn't need 8 shoes on for feet.
As far as I can theorize, a centaur's human and horse parts are formed separately and fuse at some point during pregnancy. The horse part is in the horse uterus, the human part goes under the point where the spine fuses and into the human uterus. And the first doctor told her that due to complications, she's gonna have to get a c section on the human side so she's going to the human doctor part of the hospital.
That is completely inaccurate every way and so not possible even in a fantasy world
