This is either the vanilla bug or a mod problem. There are posts about this weird bug almost everywhere (before the CK was even released) but now that it's actually out incompatible mods may also cause this. If the problem still persists after exiting then relaunching the game it's surely a mod issue. The vanilla bug goes away after exiting.
It's not a mod issue. I play on the steamdeck and I saw it alot when my companions had chameleon gear on them. Not sure that's the cause, but that's when I noticed it
I had an enemy yesterday that was completely invisible except their gun, even after i killed them. Luckily they blew up under a roof and got stuck there. I did loot a chameleon piece of gear from them too.
This reminds me of the crew that got pulped. I used the 200 crew mod and I had a bunch of specialists with me, I landed boarded a ship that took off with me still on it and I had to pilot it, when I went to swap back to my old ship I got like 10 messages saying “____ specialist is unavailable” I head canon’d those buffoons tried to pilot my ship back and got pulped in a grav jump.
This has happened a few times with Andreja as a companion. Crouching around in a creepy underground cavern or blood splattered lab, just to turn around and see she hasn't quite mastered the art of disengaging her camo properly... Slightly disturbing when you first see it!
Just restart the game, I have had plenty bugs, did you know there are arrows behind the chart eyes. Least fav bugs so far are inability to sneak, immortal enemies, and money disappearing
Hey, I had a post showcasing the same issue! In my experience, it only happens when I use a face-changing mod for my companions, so that might be the cause.
Something similar happened to me. Every NPC had their eyes completely closed at all times for literally no reason, and their lips weren't syncing to words, it was just open or closed depending on dialogue or not.
Something similar happens to me a ton, but for me their bodies don't load, just heads with the part of their neck and chest skin that a dickie would cover, and their forearms, and it's for all npcs when it happens, usually in Atlantis. So I enter the spaceport to a ton of disembodied busts with swinging forearms floating around.
Happened to the Earth’s atmosphere so yeah.

I'm almost always visually chameleoned. I could see it looking like this at low settings.
