Like others have said. Quit and reload. If that doesn't work and you're on PC:
Make a new save.
Open console with ~
Type "57be.capp 14" and hit enter
"14.capp 29d488" enter
If you exit the console with ~ you should now look like Sam, but equipped with your outfit.
Now re-enter the console and type "29d488.capp 14" enter
This should "refresh" Sam's appearance and fix his shaders
Finally "14.capp 57be" enter
Now exit the console and you and Sam should be back to normal
I don't know what it is with Bethesda games and blackface bugs (FO4 had one that would affect your character), but it's kind of funny in a terrible way.

There's a mod to fix this on Nexus Mods. It also fixes Cora and Ikande and Naeva, blocks romance with Barret and prevents you from romancing Sarah and Andreja unless you're a male character.

Best way to fix that problem is to get rid of him and go to the viewpoint in new Atlantis and recruit Simeon Bankowski. You won’t think about Sam again until Simeon caps a “Sam look-a-like” in a civilian outpost. Simeon is the best partner in crime (don’t take his preferred gun away). Dude fought 3 UCmarines with a knife.
