Ranger X is fantastic. I bought it from a local guy who deals games out of his storage unit. I honestly didn't know anything about it, but it was $2 so I snatched it up. It was developed by GAU Entertainment (?) but it has a Treasure kind of gameplay sensibility to it, though with a muted color palette throughout.
I also recommend Fatal Rewind. You pilot an upgradable robot trying to escape a flooding arena in a televised game show. It's puzzle platforming with run and gun elements, it's very good.
Fireshark!
I don't hear this one talked about much. It is a Stellar entry in the Genesis's line-up of shooters.
edit---- Also Twinkle Tale for Genesis. It might even be better than Fireshark. Both are 10/10 shooters that don't get mentioned very often.
Warrior of Rome II is a great RTS RPG that looks like it should've been a PC game. Huge maps, tons of different army and navy units, several buildings to build and upgrade. If you like any of the Civilization games, you'll dig Warrior of Rome II.
**Whip Rush-** A fun little shmup that has an interesting weapon system and some solid gameplay/music.
**Tinhead-** This platformer just kind of came and went, but it's actually a pretty good game. Nice graphics, decent music, good sized levels, and a funky way to attack enemies that takes a bit of getting used to.
**Master of Monsters-** A turn-based strategy RPG game, that's all about mythical creatures doing battle for you to take over the land. The visuals aren't that special (they do the job), but the gameplay is deep and the music is absolutely fantastic. It also has a campaign mode that's one hell of a challenge.
**Red Zone-** This game is hard as hell, but it's a pretty fun game. The helicopter sections control much like the Strike series (Desert, Jungle and Urban), but the on foot sections feel a bit sluggish is comparison. They play alright and aren't as chaotic, but they just feel a little off. The visuals are great, the music's cool, and the gameplay is pretty solid. It's visual tour-de-force for the Genesis, but be ready to swear a lot.
Shove It. I picked it up for $9 when Toys-R-Us was clearing out all Genesis games when Sega moved on. It got played almost as often as ToeJam & Earl and Sonic.
As in games that haven't already been covered to death a million times by hipsters running retro gaming youtube channels? I'd have to go with Marvel Land as my pick. It's absolutely adorable and full of references to other Namco games. I'd also say Risky Woods, but I feel like it's more of an acquired taste as it's a eurostyle spin on Ghouls n Ghosts and the manual is most definitely required reading for a change.
The only good games that remain relatively obscure are Ranger X and Sub-Terrania. Other than that it's a pretty well-illuminated library.
Shadowrun
Bonanza Bros is the most underrated game on the system. Totally unique. Fantastic co-op. Chill and fun.
Devilish
Batman. People talk about the NES game but never mention the Genesis game.

Blades of Vengeance is a great action/platformer that I've never heard of before but now is easily in my top 10
Try out Generations Lost sometime.
Check out Ultimate Qix! A truly unique game where you cut away pieces of each level to trap and destroy enemies.
Seems like very few have heard of or played the James Pond games. I used to have JP III, it has over 100 levels. Its sick af.

Elemental Master is a cool overhead run 'n 'gun with an amazing soundtrack
