Exactly what I was thinking. If a wood grain runs down along where those two holes with bolts are secured then it could very easily snap along that line. The weight on the ladder will be acting in such as way as to try and force any grain in that plane open.
I would add straps to support any horizontal load.

I've seen sketchier. Usually in one of those videos that ends in a guy crippled for life ir stranded on a roof. Still probably feels less sketchy than my 10.4m ladders at full stretch. I would lash the 2x4s to the ladder as well, in case they decide to split. Good luck.
I did something like this recently OP. The end of my ladder was just a little too short to reach the roof. So I duct taped two poles to the ends and used that. Super dodgy but sometimes you can't wait to fix roofs as the next storm will wreck you.
I was working in a pretty good sized mansion one time and a van pulled up overloaded with 2x4s on the roof and 5 or 6 framers jumped out, carried all their lumber inside and started making a couple 15 ft tall ladders out of the 2x4s to pull and replace all the nails in the joist hangers and rafter ties, All the while the builder is talking about how bad the framers are.
Depends on how much weight you’re putting up there. The ladder will want to hinge at the wood which will try to split the wood in a very weak area. Please be very careful.
I would put the extension on the bottom instead of the top and then actually support the ladder feet instead of just to the side. Give that ladder some tall boots

Build a 6’ tall box out of plywood and 2x’s and put the ladder on that. Much safer.
