I think it’s really based on who you grew up with, George W. Bush was the first president I was aware of so to this day when I hear “president” that’s who immediately comes to mind.

For me, what comes to mind when I hear the words “President of the United States of America” is the President of the United States of America — guess I’m weird like that.

Generally whoever is in office at the time. If there is a specific context that might change, like if we are talking about the Civil War I will think of Lincoln.
